The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For patients with dental insurance, we will submit your dental claim, electronically, and accept the assignment of benefits.  We ask that your co-pay, deductable and/or estimated portion be paid at the time dental services are rendered.  If there is any remaining balance after the insurance benefit is received, we will send an invoice payable, to you.

For our patients who have no dental insurance, payment is expected upon completion of your scheduled dental service(s), unless other arrangements have been made.  For your convenience, we accept Visa and Mastercard.  We also offer financing through CareCredit and Diamond Federal Credit Union
    

 
Dental Insurance

We make every effort to keep your dental care affordable while maintaining the highest level of dental care.  We accept most dental insurances.   We understand that dental insurance is a contract that is negotiated between our patients and their employers.  We will not interfere with that relationship, but will attempt to maximize your dental insurance benefits.  It is understood that dental insurance is meant to help defray your dental treatment costs by paying only a portion or percentage of the bill.  We coordinate traditional insurance with your treatment, whenever possible, and help our patients maximize any insurance benefits they have.  We have a philosophy of treating our patients with our professional judgment and experience.  This philosophy is not compatible with the restrictive nature of managed care dental plans (HMO’s).  Because of this, our office is not a contracted provider with any HMO or DHMO plans.
